Here's some SMD+SWD plugins for VGMToolbox's "Advanced Cutter/Offset Finder": http://www.sendspace.com/file/sspx2n . Just throw them in the directory listed in the info box for that tool and refresh your plugin list.
Like bxaimc told you, SMD+SWD are Procyon Studio's sequence and sample data.
Not playable by VGMStream (and won't ever be, since, you know, sequences). You can try rebuilding another NDS rom that uses this driver and doesn't have the data packed in a custom datafile, switching one of the menu song files to those from Shiren 5 and recording either from line-in or an emulator.

Looks like someone could make an alternate testpack.nds based on this driver for this new sequence format. Just suggesting.
