Extracting .STR & .STH Audio Pairs by (!!!!!) at 9:04 PM EDT on October 12, 2012
I've found an incomplete of Pac-Man World 3 (PS2) & House of the Dead Overkill (Wii) which both use .STR & .STH pairs but no way to extract them from binaries. For anyone who has successfully extracted these or knows how to, how did you do it?

Based on the sample you provided it looks like the STH file begins at 0x2800 or 0x3000 bytes from the end of the file and has size 0xE0.

The STR part begins at 0x800, with size 0x780000. I seem some possible pointers to that in the header, but not sure if it is a coincidence or not.

Also see some file names at the end of the file, but I'm not sure if this is a bad cut or has some other meaning.
